- All x-rays, CT scans, and MRI's. Please bring the actual scans either on film or on a CD. This will greatly help our doctors ability to evaluate you
- our doctor's written referral, if you have one
- A list of all your medications
- Your insurance cards and identification
- Your doctor's address and phone number
